Informed Consent, Release Agreement, and Authorization and General Information/Health History Form

This two-part form is required every year for ALL scouts, any parent(s) / guardian(s), or other family members (including siblings) attending overnight camping trips.

Submission Instructions

Once you complete the form, you have two options for submission:

• Print and Hand-in: You can print a hard copy of the form and submit it directly to the Cubmaster during pack meetings. This is the most private method to submit forms.

• Online Submission: You can submit a PDF or image of your form directly to pack administrators here. Digital copies are not kept long term.
