Informed Consent, Release Agreement, and Authorization and General Information/Health History Form

This two-part form is required every year for ALL scouts, any parent(s) / guardian(s), or other family members (including siblings) attending overnight camping trips.

Submission Instructions

Once you complete the form, you have two options for submission:

• Print and Hand-in: You can print a hard copy of the form and submit it directly to the Pack Committee Chair during the next Pack 134 meeting. This option is preferred if you want to ensure confidentiality, as the form will not be shared digitally.

• Email: You can scan or take a clear photo of the completed form and email it to Pack 134's group email address at decaturpack134@gmail.com. Please be aware that this is a group email, and multiple leaders will have access to the information.
